Housing Support Program (HSP)
The Housing Support Program follows Rapid Rehousing and Progressive Engagement models. HSP services are provided to families with children who are receiving public assistance and are eligible for Cal-WORKs. Participants must be literally homeless, which is defined as, “lacking a fixed or regular nighttime residence, are in temporary shelter, or are residing in a place not meat for human habitation.” Families enrolled in HSP are assessed based on their level of crisis and their level of vulnerability.
The Housing Support Program can only be accessed through a referral from CalWORKs+.
HOUSING DISABILIITY AND ADVOCACY PROGRAM(HDAP)
The Housing Disability Advocacy Program (HDAP) assists individuals and families experiencing homelessness who are likely eligible for disability benefits. Through HDAP, participants receive case management services to assist in housing navigation and the disability application process. Additionally, HDAP may also assist individuals at risk of becoming homeless who are likely eligible for disability benefits.
The Housing Disability Advocacy Program can be accessed through a referral from Social Services General Assistance Program.
TC Bridges Program
Wraparound is a voluntary, strength-based approach, which helps youths and their families become stable. This is accomplished through team meetings where the family and the Bridges team collaboratively create and individualized plan that is based upon the family’s needs. Wraparound is driven by the 10 principles and is delivered through activities in four concurrent phases. Wraparound ultimately is not a program, it’s a process.
The Bridges Program can only be accessed through a referral from Child Protective Services.
